External Access

If you are not a user here at the University of Michigan, there are two forms that you will need to address, in addition to the on-line CAC form.

These forms need to be signed and mailed to us (Classic Mail, not e-mail). If you are in a rush, we will accept a fax of these forms with the understanding that the originals will arrive shortly via post. They can be faxed to the attention of Andrew Caird at (734) 936-3107.

Form #1 (PDF) is a 'Fair Conditions of Use & Confidentiality' agreement in which you promise that you won't steal or damage anything and that you'll be a good-hearted user of our resources.

Form #2 (PDF) is an account application which requests the user information needed to create your account (user ID, password, etc).
